The Czech Republic Passenger Locator Form is an international screening travel document designed mainly to gather information from arriving international visitors to the Czech Republic. This document will aid track any potential illness entering the country through personal inspection. It is your "IMMERSE" travel guide. Please only use it once! Remember, it doesn't replace any travel documentation.

The Czech Republic passenger locator forms are designed to collect information from travelers who arrive in the Czech Republic for personal or tourist purposes. First, the name, complete birth date, current mailing address and contact number if different, passport type (national or international) and country of birth. If you have a different country of origin, please use the correct translation (please do not confuse!) Next, enter your arrival address. Please see the table below if you are traveling within the EU. The second row will be for your destination.

The second item on the list of questions is a short explanation about your need for a passport. If you're traveling between the EU and the USA or Canada, you'll need a visa to enter the country. This is commonly known as a COVA (or visa card). You must be aware that without a visa you will not be allowed entry at all! You must fill out the Czech Republic passenger locator form accordingly.

Within two weeks, you will receive your certificate of accomplishment after you have completed the test and submitted it. This certificate will allow you to travel anywhere in the world. Once you have your certificate, you will need to fill out a Czech Republic passenger locator sheet and bring it to the airport. The forms are readily available at the airport and can be printed out after you pass through the security checkpoint. While you're waiting for your flight, make sure to get in a cab to fill out the registration and insurance cards.
